Ordering Reports

For Residents/Owners:

Order the report by one of the following options:

  1. Calling the SAFD Records Management Division between 9:00 AM and 4:00 PM Central Standard Time (CST). To locate a report, you must supply the following information:
    • a. Date of the fire.
      b. Physical address of the location of the fire.
      c. The address the report will be mailed to, if you are choosing that delivery method*.    
  2. Completing the Fire Incident Report Request (PDF) form and:
    • a. Fax it to (210) 207-4969 or
    • b. Mail it to:

  3. Reports can also be requested in person at the same address above at the Fire Department Records window on the first floor between 9:00AM and 4:00PM CST and are typically ready within 30 minutes. Directions to SAFD Headquarters from Google.

There is no charge for a single copy of a Fire Incident Report. Fees may apply for multiple copies. You will be advised of any costs prior to creation of the reports.

For Private Companies:

Order the report by:

Complete the Fire Incident Report Request (PDF) form and:

  1. Mail it, along with the appropriate fee of $5.15 (check or money order) based on the SECTION 10, and a self-addressed and stamped envelope to:

For Other Organizations/Agencies:

Order the report by:

Complete the Fire Incident Report Request (PDF) form and:

Notes:

  • The report can then be faxed, mailed or picked up within two working days.
  • Requests for a Fire Incident Report that involve a fatality are forwarded to SAFD’s Human Services Section for additional review and handling. In such cases, Human Services will follow up with the requestor accordingly.
  • Arson Fire Reports should be ordered at the SAPD Records Division.
For Patients:

You may pre-order the report by calling the SAFD Records Management Division at (210) 207-4974 / (210)-207-4975 between 9:00 AM and 4:00 PM Central Standard Time (CST).

Have the following information ready:

  1. Patient’s name.
  2. Patient's date of birth.
  3. Patient’s social security number.
  4. Date, time, and location of the service.
  5. Requestor’s name and contact information.
If picking up the report:

Reports can be picked up at the SAFD Headquarters building between 9:00AM and 4:00PM CST at the Fire Department Records window on the first floor. Directions to SAFD Headquarters from Google.

  1. If you are the patient listed on the report, you will need to show a picture ID and complete the Patient Authorization for Disclosure of Health Information (PDF) form.
  2. If you are not the patient listed on the report, you will need to show a valid “Power of Attorney” document or a signed Patient Authorization for Disclosure of Health Information (PDF) form.
  3. A fee of $7.50 (cash, check or money order) based on the SECTION 10 is required.
If having the report mailed:
  1. Complete the Patient Authorization for Disclosure of Health Information (PDF) form.
  2. Mail the form, along with:
    • a. a copy of your driver license or state issued ID
    • b. the appropriate fee of $7.50 (check or money order, please no cash) based on the SECTION 10, and
    • c. a self-addressed and stamped envelope to:

Notes:

If you are not the patient listed on the report, you will also need to provide a copy of a valid “Power of Attorney”, or the Death Certificate if the patient is deceased.

All checks should be made payable to the City of San Antonio Fire Department.

For Law Offices and Records Retrieval Companies:
  1. Complete the EMS Records Request Form and mail it to:

  2. Fees and the checklist of all the items required to be submitted with your request are listed on the form.
  3. The request may also be hand-delivered at SAFD Headquarters building between 9:00AM and 4:00PM CST at the Fire Department Records window on the first floor. Directions to SAFD Headquarters from Google.

Note: Requests are NOT accepted by FAX.

For Public/Medical Organizations:
  • Call SAFD Records Management Division at (210) 207-4974 / (210)-207-4975 between 9:00 AM and 4:00 PM Central Standard Time (CST).
RECORDS FEES:

SECTION 10. Sections 11-19 of the City Code of San Antonio, Texas.
The fees for duplicating and preparing records of the emergency medical services division of the fire department shall be that amount authorized to be charged by the provisions of Chapter 552 of the Texas Code.

Effective November 12, 2012, the charges for reproduction and preparation of patient records are as follows:

  1. $15.00 per hour
  2. $ 6.00 notary fee
  3. .10 per page

Billing

The EMS Division’s Medical Intensive Care Ambulances are supplied with state-of-the-art equipment and are essentially an emergency room on wheels. An established city ordinance requires EMS services be billed directly to the patient. This ordinance helps to ensure the availability of special services, such as EMS, by making them self-sustaining.
