BICYCLIST/PEDESTRIAN ACCIDENT REPORT FORM (DPS) (PDF) The Texas DPS is required to gather statistical information concerning accidents involving bicyclists and/or pedestrians. DPS Form TR-11 is designed to be a self-reporting form, that allows the bicyclist/pedestrian to complete the form and mail it directly to DPS in Austin. PLEASE NOTE: This DPS form is to be completed IN ADDITION TO the regular SAPD Accident Report. Persons involved in a bicyclist or pedestrian accident should download the following DPS forms: FORM TR-11: Bicyclist/Pedestrian Accident Report Form INSTRUCTIONS for Completing FORM TR-11 Please complete FORM TR-11 and mail to: Bea Taylor, Training Academy, Texas DPS, PO Box 4087, Austin, TX 78773-0252.

POLICE REPORT/ACCIDENT REPORT REQUEST FORM (PDF) PDF Form to request a Police Report or Accident Report. Please print out this form, fill in the appropriate spaces, and bring it with you to request a Police Report or Accident Report. (The form is small.) WHERE: Reports are available at the Main SAPD Headquarters Building (214 W. Nueva) and at Community Links Centers at Las Palmas, Valley View, South Park and McCreless. BY MAIL: If you submit the request by mail, please include a money order for the appropriate fee. SUPPLEMENTARY PROPERTY LOSS REPORT FORM (PDF) PDF Form to report items lost due to a reported crime such as a burglary, robbery or theft. (An Offense Report must have already been made and a Case Number assigned.) This form is used by the victim to report lost items that were not included in the original Offense Report. FORGED CHECK FORM(PDF)    FORGED CHECK FORM(MSWord) To prepare a Forged Check Form for submission to the SAPD Forgery Detail, you may use either of two formats:    PDF : Print out the Form in PDF format, type or hand write in the requested information, staple the check to the form, and submit to the SAPD Forgery Detail.    MS Word : Bring this Form up in your web browser, type in the appropriate information in the shaded blanks, then print out the completed form, attach the check, and submit to SAPD Forgery Detail. This format allows you to use your computer to fill out the form before printing.

CREDIT CARD ABUSE FORM(PDF)    CREDIT CARD ABUSE FORM(MSWord) THIS FORM IS FOR USE BY COMPLAINTS FROM OUTSIDE OF SAN ANTONIO ONLY. Please contact the SAPD Forgery Unit if you are making a complaint from within San Antonio.
