This form must be filed by persons or entities seeking award of city-funded discretionary contracts to disclose ownership, lobbying and political contributions.

DISCLOSURE OF BENEFITS

If a person who requests official action on a matter knows the  action will confer an economic benefit on any city official or employee distinguishable from the effect it would have on members of the public in general, he or she must disclose that fact unless the interest of the city official or employee in the matter is apparent.

Form for disclosing conflict of interest and documenting recusal by a city official or city employee.

LOCAL GOVERNMENT OFFICER CONFLICTS DISCLOSURE STATEMENT

This is the notice to the appropriate local governmental entity that a local government officer has become aware of facts that require the officer to file this statement in accordance with Chapter 176, Local Government Code.

CONFLICT OF INTEREST QUESTIONNAIRE

By law this questionnaire must be filed with the records administrator of the local governmental entity not later than the 7th business day after the date the person becomes aware of facts that require the statement to be filed. See Section 176.006, Local Government Code.

THIRD PARTY SPONSORED TRAVEL

Any person who, in connection with his or her official duties, accepts a trip or excursion involving the gratuitous provision of transportation, accommodations, entertainment, meals, or refreshments paid for by a person or entity other than a public agency must file with the City Clerk, before embarking on the travel, a disclosure statement.

Form to be filed by certain city officials and staff to report gifts received by relatives.

SWORN COMPLAINT OF ETHICS VIOLATIONS     

Any person (including a member of the Ethics Review Board or its staff, acting personally or on behalf of the Board) who believes that there has been a violation of the ethics laws may file a sworn complaint with the City Clerk.
