Thank you for choosing to submit your application for appointment. We recommend that you ensure that you have the latest version of the browser prior to completing the application. PLEASE READ ALL OF THE INSTRUCTIONS BEFORE PROCEEDING.

Please select the board or commission you would like to apply for from the drop down listing. Other fields contain drop down boxes or merely require a Yes or No selection.

Press the Submit when you have completed the application. You will receive a confirmation showing your selections. If you would like to apply for additional boards, use the Back function of your browser and select the title of the other board from the listing and press the Submit button again. You will receive another confirmation showing your selections.

We recommend that you print a copy of the application for your records or, using the File... Save As function of your browser, save this page to your computer, or both. You may include additional information by mail if  you send in the application.

Some boards require that applicants satisfy certain requirements to fill categories of vacancies on these boards. You should visit the Vacancies page to review the vacancies for the boards and determine if certain categories apply. The category, if any, is located in the extreme right column of the page. You should enter this category in the Category If Applicable block on the application. If a category does not apply to the board you have chosen, please leave this block blank
