Healthy Start

Case Management

Healthy Start case management services provide home visits for pregnant and postpartum women, their families and children through 2 years of age.

Case managers may help with:

  • Small Incentive items
  • Education/School Issues
  • Setting individual goals

Home visits are provided to:

  • Assess the needs and develop a plan for pregnant/postpartum women and their family.
  • Locate services in the community, such as medical services.
  • Make appropriate community referrals, such as mental health and substance abuse.
  • Provide follow-up with families to ensure that needs are met.
  • Provide health education services on topics, such as healthy pregnancy and the baby's development.
  • Assist with prevention of child/family abuse and domestic violence.
