Outreach
The Healthy Start outreach team actively recruits and offers education to potentially eligible families within the targeted zip codes. The team also provides home visits to low-risk clients, their families and children through the age of 2.
Outreach home visits consist of:
- Prenatal Education
- Infant Care Education
- Linkage to community resources
