Provides routine immunization services to adults.

NOTE:

• Please bring a current Immunization Record to every visit.
  
• Clients must bring a current Medicaid/CHIP card to receive immunizations at no cost.
• Vaccines For Children (VFC) eligible clients administrative fee is $14.85 per immunization. (excluding adult immunizations).
• Clients with private insurance: current fee-for-service cost of each vaccine plus $15.00 administrative fee per visit.
• Current price list is posted at each clinic location.
• Immunization records may be purchased for $5.00 per record.
• Method of payment may include: Cash, Check or Credit Card.  Checks and credit cards will be accepted with valid proof of identification.  Third party checks will not be accepted.
