Forms of Identification
Proof of Eligibility
Each applicant must present a valid primary form of photo identification along with their application. If the applicant is unable to present a valid primary form of photo identification, two secondary forms of identification are required - one of which bears the applicant’s signature.
Primary Forms of Identification

2. Federal/State/County/City ID
3. Student ID
4. Military ID
5. Employment Badge or Card
Secondary Forms of Identification
1. Voter’s Registration Card
2. Social Security Card
3. Clinic or Health Insurance Card
4. Hunting or Fishing License
5. Credit Card
6. Library Card
