BIRTH
CERTIFICATES
The City of San Antonio maintains copies of birth records
from 1897 to the present. (Additionally, the Vital
Statistics Office of Bexar County located at County Courthouse
at 100 Dolorosa Street in downtown San Antonio has some birth
records on file from 1837 to 1967.) Birth records are confidential
for the first 75 years after birth. During this period, only
qualified applicants can obtain copies of these records. A
qualified applicant is the person named on the birth certificate
or an immediate family member, either by blood or marriage,
including a step-parent, step-sibling, guardian or legal representative.
You can obtain copies of birth certificates in person from
the Metropolitan Health District Office at 719 Santa Rosa
Street, in downtown San Antonio. The office hours are 7:45
AM through 5:00 PM, Monday through Friday. You will need to
fill out a form, pay a $23 fee and show a valid driver's license
or identification card. You can also obtain a birth certificate
by mail.
To obtain a birth certificate by mail:
1. Download the form and fill in required information from
the
Vital
Statistics website.
2. Enclose a check or money order for $23 made out to the
City of San Antonio. There may be additional charges if you
do not know the year of the birth.
3. Enclose a photocopy of your driver's license or identification
card
4. Mail form, check and photocopy of license or identification
card to:

DEATH
CERTIFICATES
The City of San Antonio maintains copies of death records
from 1873 to the present. (Additionally, the Vital
Statistics Office of Bexar County located at the County
Courthouse at 100 Dolorosa Street in downtown San Antonio
has some death records from 1903 to 1967.) Death records are
confidential for the first 25 years after a death. During
this period, only qualified applicants can obtain copies of
these records. A qualified applicant is the person named on
the birth certificate or an immediate family member, either
by blood or marriage, including a step-parent, step-sibling,
guardian or legal representative.
You can obtain copies of death certificates in person from
the Metropolitan Health District Office at 719 Santa Rosa
Street, in downtown San Antonio. The office hours are 7:45
AM through 5:00 PM, Monday through Friday. You will need to
fill out a form, pay a $21 fee for the 1st copy and $4.00
for each additional copy of the same certificate ordered with
the certificate request fee and show a valid driver's license
or identification card. You can also obtain a death certificate
by mail.
To obtain a death certificate by mail:
1. Download the form and fill in required information from
the
Vital
Statistics website.
2. Enclose a check or money order for $21 made out to the
City of San Antonio. If you wish to receive additional copies
in this same order, the additional copies will cost $4 each.
There may be additional costs if you do not know the year
of the death.
3. Enclose a photocopy of your driver's license or identification
card

MARRIAGE
CERTIFICATES
The City of San Antonio does not
maintain or possess copies of marriage certificates.
To obtain a copy of a marriage license certificate please
contact the County Clerk in the county in which the marriage
license was obtained. For marriages in Bexar County from
1964 to the present, you may find more information about
getting a copy of the marriage license

For marriages in Bexar County which took place before 1964,
you will need to go in person to the Office of the County
Clerk at 100 Dolorosa Street in downtown San Antonio, to research
the license records.
IMMUNIZATION
RECORDS
The City of San Antonio maintains Immunization Records from 1978 to the present.
A qualified applicant is the person named on the Immunization record or an immediate
family member, either by blood or marriage, including stepparent, step-sibling, guardian,
or legal representative.
You can obtain copies of Immunization record in person from one of the
Community Link Centers
or one of the
clinics.
To obtain an Immunization record by mail:

2. Enclose a check or money order for $5 made payable to the City of San Antonio
3. Enclose a photocopy of your driver's license or id number.

DIVORCE
RECORDS
The City of San Antonio does not
maintain or possess copies of records pertaining to divorces.
Divorce records are filed with the court system in which the
divorce took place. For divorces which took place in Bexar

COURT
DOCUMENTS
City of San Antonio Municipal Court:
The City of San Antonio Municipal Court handles traffic matters,
City of San Antonio code compliance violations and class C
penal code violations (assaults, disorderly conduct, theft,
minor in possession, minor consuming, possession of graffiti
implements, drug paraphernalia and curfew violations)
for violations which took place within the City of San Antonio
city limits. Request San Antonio Municipal Court
records online, by
mail, by fax or in
person.
For traffic, code compliance or class C penal code
violations outside the City of San Antonio city limits,
you must contact those municipalities, including Balcones
Heights, Live Oak, Alamo Heights, Olmos Park, and Terrell
Hills.

Local Criminal History
To obtain local criminal history records from the San Antonio Police Department, please come in person to 214 W. Nueva
Street in downtown San Antonio with picture identification.
Individuals with a criminal history will be asked to submit
a thumbprint for verification. For more information on
letters, background checks, fingerprinting, notary, and
certification call 207-7605.

DEPARTMENT OF PUBLIC SAFETY
CRIMINAL HISTORY RECORDS
To obtain state criminal history records from the Department of Public Safety, an individual or their authorized representative must submit a written request for the Criminal History Record Inquiry (CHRI). If an authorized representative is the requestor, written authorization from the individual must be attached. The following list includes all of the items that must be submitted with each written request for CHRI:
- Printed name of individual. Include any other names used.
- Sex.
- Race.
- Date of Birth: Month, Day, Year.
- A complete and legible set of fingerprints on DPS-approved fingerprint cards.
- Social Security Number.
- Return address of individual or return address of person the individual has authorized to receive the criminal history record.
- Enclose check, or U.S. money order for $15.00 per each fingerprint card.
Any missing item(s) will cause the request to be returned.
NOTE: All local police and sheriff’s departments are supplied with DPS approved fingerprint cards. Fingerprints submitted to request access to criminal history records must be taken by a law enforcement agency. Most departments will provide the service. However, it is the responsibility of the individual or their authorized representative to mail the completed card to DPS in Austin, along with other necessary information. Mail the written request, fee, and the fingerprint card to:

FEDERAL BUREAU OF INVESTIGATION (FBI) CRIMINAL HISTORY RECORDS
To obtain national criminal history records, the individual must prepare a signed written request for a copy of their record. Include your complete mailing address. If you have a deadline please include the deadline in your written request and on the outside of the envelope. Obtain proof of identity, which consists of a set of your original fingerprints (no copies), with your name, date of birth and place of birth. Fingerprints should be placed on a standard fingerprint form (FD-258). Send fingerprint card along with $18 in the form of a money order or certified check (no personal checks or cash) made payable to the Treasury of the United States. Be sure to sign where required. Mail the written request, fee, and the fingerprint card to:
