1. DEADLINE: ALL NNO2009
REGISTRATION FORMS
MUST BE SUBMITTED
BY WEDNESDAY, SEPTEMBER 23, 2009(IF ON-LINE OR FAXED) OR
ARRIVE BY SEPTEMBER 23, 2009 (IF MAILED).
2. COMPLETE ALL QUESTIONS: Please fill out all questions on the Registration Form as directed. Continue
to scroll down through the form to complete the 12 required questions.
3. CITY COUNCIL MAP: If you do not know your Council District, please go to the
MAP of
Council Districts and locate your neighborhood and its district. You can ZOOM
in on sections of the map by using the Magnifying Glass icon (click on icon, then
click on area of map you want to enlarge). For a list of Council Districts
and Council Persons, please go to:

4. FOR MULTIPLE ACTIVITY LOCATIONS: If your group is having
activities at multiple locations, please SUBMIT A SEPARATE ON-LINE REGISTRATION
FORM for EACH LOCATION.
5. TIPS FOR ACTIVITIES: If you need ideas for "Activities", please print out the
"NNO TIPS" pages.
