Ordering Fire & EMS Reports |
| For EMS Reports |

Pre-order the report by calling the EMS Administrative Offices at (210) 207-7525 between 8:00 AM and 4:30 PM Central Standard Time (CST).

Have the following information ready to give to the EMS Administrative Office:
- Patient’s name.
- Patient's date of birth.
- Patient’s social security number.
- Date, time, and location of the service.
- Requestor’s name and contact information.

The fees for duplicating and preparing records of the emergency medical services division of the fire department shall be that amount authorized to be charged by the provisions of Chapter 552 of the Texas Code.
Effective November 12, 2012, the charges for reproduction and preparation of patient records are as follows:
$15.00 per hour
$ 6.00 notary fee
.10 per page
All checks should be made payable to the City of San Antonio.

| For FIRE Reports |
- Order the report by calling the Fire Prevention Offices at (210) 207-8410 between 8:00 AM and 4:30 PM Central Standard Time (CST). The report can then be mailed or picked up within two working days.
- Reports can also be requested through the mail by sending a letter to:

The report can then be mailed or picked up within two working days.
- Reports can also be requested in person at the Fire Prevention Offices at 1901 S. Alamo, and are typically ready within 30 minutes. Driving directions to the Fire Prevention Offices from Google
- To generate a report, you must supply the following information to the Fire Prevention Office:
- Date of the fire.
- Physical address of the location of the fire.
- The address the report is to be mailed to, if you are choosing that delivery method.
- There is no charge for a single copy of a Fire Incident Report. Fees may apply for multiple copies. You will be advised of any costs prior to creation of the reports.
- Please Note: Requests for a Fire Incident Report that involve a fatality are forwarded to SAFD’s Human Services Section for additional review and handling. In such cases, Human Services will follow up with the requestor accordingly.
