On March 1, 2004, the San Antonio Police Department began asking drivers
to fill out and submit to the Texas Department of TRANSPORTATION (TxDOT - no longer handled through DPS) the
"Blue Form" (CR-2), when the accident DID NOT involve injury to any party AND
when the DAMAGE was less than $1000.00 to the property of any one person.
You may obtain a "Blue Form" "CR-2: Driver's Crash Report Form" from any
SAPD Substation, or you may download one from the TxDOT web page, linked
below. Please read the Instructions Sheet carefully before completing and
submitting the Form.
