Click on the “Claim Form” link and follow the directions on the form. All claims against the City must originate from the City Clerk’s office.

The City’s Office of Risk Management will retrieve your Claim Form, enter it into our system, and assign it to a General Liability Claims Adjuster, who will investigate and evaluate your claim. You will receive a call from the General Liability Claims Adjuster within 5-7 days of filing a claim.

All accidents involving City vehicles, whether on duty or not, must be reported to the police and your supervisor immediately.

If you are driving your personal vehicle, show the police your personal insurance card. If you are driving a City vehicle, show the police the self-insured information card located in the glove compartment.

You are required to report your injury or accident to your immediate supervisor as soon as possible.

You have the right to choose any doctor who will accept Workers’ Compensation. We encourage all employees to seek medical treatment through the City’s Employee Health & Wellness Center. This is a fully staffed medical facility that serves employees of the City of San Antonio only.

You may contact the City’s third-party administrator, TRISTAR Risk Management Services, at 210.404.0400.

This is a state-funded Agency.  Their responsibilities are to:

  • Promote safe and healthy workplaces;
  • Provide customers with information about their rights and responsibilities;
  • Ensure the delivery of appropriate benefits;
  • Ensure appropriate and efficient health care for all injured employees and fair and reasonable reimbursement for health care providers;
  • Assist in timely returning injured workers to productive roles in the Texas workforce;
  • Assist in resolving disputes between the employer and employee.

If it is a severe injury requiring immediate medical treatment, go to the nearest emergency room or call 911. Notify your supervisor as soon as possible of the injury. The supervisor must report all work-related injuries directly to the City’s third-party administrator, TRISTAR Risk Management, in one of three ways:

After an injury occurs, you must choose your treating doctor from the IMO Med-Select Network provider list which can be found online at and click "Find a Provider" or you may call a network customer care representative for assistance at 1.888.466.6381 or 1.877.870.0638.

The physician should provide you, and the City, with a form titled Texas Workers’ Compensation Work Status Report Form, DWC 73. Notify your immediate supervisor that you have been placed off work and submit the DWC 73 form to the Human Resources Specialist (HRS) the next business day.

The physician should provide you, and the City, with a form entitled Texas Workers’ Compensation Work Status Report Form DWC 73. Hand carry the Work Status Report to your immediate supervisor for determination of job placement.
